Money Follows the Person (MFP) Waiver
The Money Follows the Person (MFP) Waiver helps individuals transition from institutional settings (such as nursing homes or hospitals) back into their own homes or community-based settings.
Program Eligibility
For Care Recipients
- Be a Medicaid beneficiary
- Have lived in a Medicaid-approved institution (i.e. hospitals, psychiatric residential treatment facilities, nursing homes or other long-term care facilities)
- Meet the institutional level of care
- Transition to a qualified residence
For Caregivers
- Family members (except spouses/legal guardians) may qualify as paid caregivers.
- Must complete required caregiver training and pass background screenings.
- Should be able to provide assistance with daily living activities to the participant.
