SFC Participant's Requirements for Providing Care At Home
You are eligible for the SFC Waiver Program if:
- You're medicaid eligible
- You're a participant in the CCSP or source waiver program
- You need five or more hours of care
- You need help with activities of daily living (ADLs)
Caregiver Requirements:
- Must be a family member of at least 18 years of age
- Must be committed to taking care of a member
- Must live with the person as the primary caregiver
- Must be biologically related to the member
