Overview
The Georgia Pediatric Program (GAPP) is a Medicaid-funded initiative by the Georgia Department of Community Health (DCH) that delivers in-home skilled nursing and personal care services to medically fragile children under 21 years old. This program is designed to help children with complex medical needs receive professional care from the safety and comfort of their own home — at no cost to families.

Program Eligibility
For Care Recipients (Children)
- Medicaid Coverage: Must be an active Medicaid recipient in Georgia.
- Age: Must be under 21 years old.
- Medical Need: Must have a condition that requires skilled nursing care or personal care assistance.
- Physician-Ordered Services: Must be under the care of a physician who recommends in-home care services.
For Caregivers (Parents/Guardians)
- Must be involved and knowledgeable in the child’s care needs.
- Must participate in the planning and coordination of services.
- Must provide a home environment suitable for in-home healthcare delivery.
- Must collaborate with our care coordinators to complete and submit required documentation.
